In consultations at the clinic, I often hear similar complaints: "The area under my eyes looks hollow, but fillers scare me; the lines on my forehead bother me, but Botox feels like too much." There are also many people who are interested in skin boosters yet hesitate for a long time in front of the treatment chair because they worry, "What if it clumps or forms nodules inside my skin?"
So today I would like to look at this concern from a slightly different angle. This is about Re2O, a third-generation ECM booster designed to replenish the structure within the skin itself while easing the worry about nodules that has often come up with existing skin boosters.

Why dermatology trends are moving from stimulation to restoration
If I were to sum up the current direction of skin treatments in one sentence, it would be that the center of gravity is shifting from "stimulation" to "restoration." Why is that? The answer lies in "rebar and concrete."
What determines the elasticity and thickness of the skin is not simply the amount of moisture. The key is how sturdy the home in which cells settle and live, that is, the extracellular matrix (ECM), is. It is easy to understand if you compare the dermis to a building.
- Collagen and elastin are the rebar that supports the building
- The matrix, including hyaluronic acid, is the concrete that fills the empty space

Once aging begins, this rebar and concrete decrease little by little each year. The building sagging feebly is what we see as wrinkles, and the walls thinning is what we experience as loss of elasticity. Changes such as hollowing under the eyes and fine lines settling on the forehead ultimately start here as well.
How is it different from existing methods?
Until now, there have been two broad ways of addressing this problem. One was to fill in "water" (moisture), and the other was to stimulate the cells that make collagen through a controlled inflammatory response and put them to work.
Re2O starts from a concept that goes one step further: the idea of "putting back the rebar and concrete that have already been lost." Its key ingredient is human dermal tissue (hADM) from which the cells that could trigger immune rejection have been removed, leaving only pure structural components such as collagen and elastin, which are then processed into powder form.

Divided by generation, their characters become clear
When I explain to patients in the clinic, I divide boosters by generation and summarize them as follows. Because they do different jobs, it is better to approach them not by asking "which is better" but by asking "which suits the situation."
| Generation | Representative treatment | Working concept |
|---|---|---|
| 1st generation | Rejuran | A nutrient or tonic for tired cells |
| 2nd generation | Juvelook, Sculptra | A seed that stimulates cells to make collagen |
| 3rd generation | Re2O | Directly fills in the building material itself |
If Rejuran lends strength to cells, Juvelook and Sculptra stimulate cells so that your own tissue builds up by itself. Re2O differs in that it skips that process and puts in finished structural material.

My criteria for changing direction with thin skin
What I pay attention to when performing Re2O treatments is the safety side.
Collagen boosters such as Juvelook work reasonably well, but in thin-skinned areas such as under the eyes or the forehead, I was cautious about treatment out of concern that the ingredients might clump and form nodules. Because Re2O does not fill in collagen through a foreign-body reaction but instead puts in finished structural material, the risk of nodules in such areas is reported to be relatively low.
Even if there is something that looks like a nodule, it is usually just a simple clump of powder and tends to be relatively easy to manage. Of course, there are individual differences in response and recovery speed. If you have hesitated over skin boosters because your skin is thin, or if hollowing of the forehead or under the eyes concerns you, Re2O can be one option. I have explained a little more about how I distinguish fillers from boosters in How I look at fillers in the clinic, so it would be good to read it as well.
If you are curious about the treatment itself, you can find an overview in the Re2O treatment guide, and if you are wondering whether it suits your skin, feel free to ask via Consult via chat now.
Here are frequently asked questions, just as they are asked
Q1. Someone else's skin tissue sounds a little scary. Is it safe? Re2O uses raw material that has passed the standards of the American Association of Tissue Banks (AATB). It is a material from which the cells that trigger immune reactions have been removed, leaving only pure structural components, and the background is that this kind of material has long been used in burn treatment and reconstructive surgery.
Q2. Which is better, Juvelook or Re2O? Their purposes differ. If the goal is to restore volume, such as for hollow cheeks, hyaluronic acid filler or Juvelook Volume may be a better fit. Conversely, if you want to refine fine lines around the eyes and on the forehead and make the skin itself denser and firmer, Re2O, with less burden of nodules, tends to be more suitable.
Q3. How long do bumps or bruises last after treatment? Because Re2O is a fine-particle powder, the bumps tend to settle relatively quickly. However, there are differences depending on the injection method.
- Injection with a needle can cause bruising.
- With a cannula, bruising is often less.
- It usually looks natural within 2 to 3 days, so it tends not to interfere much with daily life.
Please also keep in mind that the recovery pattern varies between individuals.
I especially mention it to people like these
- People concerned about hollows under the eyes
- People who want to soften fine lines on the forehead
- People who want to restore firmness to skin that has become thin
- People who are interested in skin boosters but worried about nodules
- People who want to improve the density and texture of the skin itself

Today's content in three lines
- Skin aging is the process by which the building structure called the ECM gradually collapses.
- Instead of stimulating cells, Re2O directly fills in pure dermal structural material.
- Its strength is that it can be used even on thin skin such as under the eyes and on the forehead with less burden of nodules.
What is hard to judge on your own is most accurately assessed together in the clinic. If you have any questions, feel free to ask via Consult via chat now. Specific costs are explained during the consultation after your condition has been checked.
